From diabetes to digestive disorders, medical nutrition therapy can optimize health outcomes, increase patient satisfaction, and reduce the costs associated with physician time. We work alongside referring physicians and multidisciplinary health care team members to provide cost-effective and patient-centered care.

Submitting a referral is easy!

Fill out the patient referral form below and fax to 855-670-0422 along with insurance information, recent labs, and two recent chart notes.

Your patient will be contacted within 1 week for scheduling.

After your patient is seen, we will fax the assessment for your records.

Mariah Ecker, RD; Nicole Sievers, RD, CDCES; Julia Solomon, RD; and Teresa Quintero-Lopez, RD are participants in the Hawai’i Nutrition Hui organized health care arrangement (OHCA) as "covered entities". As part of Hawaii Nutrition Hui, the covered entities may share Protected Health Information (PHI) with each other as necessary to carry out any health care operations and activities of the OHCA.

Insurance benefits for nutrition counseling are better than ever! More often than not our clients are fully covered with no co-pays, co-insurance, or deductibles, which can make working with a Registered Dietitian an affordable option for your patients! The members of Hawai'i Nutrition Hui are in-network providers with HMSA, HMAA, UHA, Aloha Care, UHC, Humana, Cigna, Aetna, Ohana, Anthem Blue Cross Blue Shield plans nationwide, and Medicare.

We will verify your patient's coverage and bill their insurance carrier on their behalf. Not all nutrition related diagnoses are covered by insurance, and coverage may vary between plans. A physician referral is usually required.
